How to Book an Appointment or Make an Enquiry
We have made it quick and simple to submit a request for an appointment or medical enquiry. Our online system ensures that your query is directed to the right person at the right time.
1. Start Your Booking: Visit our online booking and triage system.
2. Complete the Online Form: Enter your details and explain your medical query.
3. Submit Your Form: Available Monday – Friday, 7:30 AM – 4:30 PM.
4. Your Form is Reviewed: A senior clinician will assess your request the same day.
5. Next Steps: You will be contacted if an appointment is needed or given self-care advice.
What Happens After You Submit the Form?
Once your form is submitted, our dedicated clinical team will carefully review your request and ensure it is directed to the right service. Our goal is to make sure you receive the best care in the shortest possible time.
• Administrative queries will be handled by the Care Navigator or Admin Team.
• Clinical queries will be reviewed by a senior clinician and prioritised based on urgency.
• If an appointment is needed, our patient support team will contact you to arrange one.
• Some issues may be resolved without an appointment through self-care advice or alternative services.
• You may be asked to upload photos where relevant to assist the clinician.
Need Help Using the Online Booking System?
We understand that not everyone has access to the internet or the ability to use an online system. At Yadava Practice, we are committed to making sure that all of our patients receive the care and support they need. If you experience difficulties with technology, have a disability, or require additional help, we are here to assist you.
• Call our reception team on 01375 390575, and our friendly staff will complete the form on your behalf.
• Visit the surgery to use our dedicated patient tablet, where our team is happy to assist you.
• If English is not your first language, we can arrange translation support.
• Elderly or vulnerable patients who need additional support should contact us, and we will ensure they receive the right care.
What to do Out of Hours?
We will be switching eConsult off every weekend, while the surgery is closed and you will be unable to send one from 4.30pm on Friday until 7.30am Monday morning. For additional care at these times please use the guidance below:
• You can use the eConsult facility to look up general medical advice.
• You can also get advice from the NHS website or the NHS App.
• Your local Pharmacist can help you with self care.
If your medical problem cannot wait until the surgery is open please contact the NHS 111 helpline by dialing 111 from any phone, or by visiting NHS 111 Online. NHS 111 online is also a good place to check your symptoms.

The Yadava Practice fully support people with hearing problems, is disabled friendly and has access ramps into the Practice. If you need any assistance please don't hesitiate to contact us.

• Visit the practice reception and use the designated patient tablet with staff assistance.
• Ask a trusted friend or family member to complete the online form for you.
• Call our Care Navigators, who will be happy to help.
• Less than 7 days: You do not need a GP note—your employer can provide an SSP form.
• More than 7 days: Submit an online request, and a clinician will assess your need for a MED3 certificate.
• Call the surgery before 10 AM to request a visit.
• A GP or Practice Paramedic may carry out the visit.
• Download and complete the SMS Consent Form and return it to reception.
• Submit a Travel Vaccination Form at least 6 weeks before travel.
• If submitted later, you may need to visit a private Travel Clinic.
• Fill out a Consent Form detailing the specific records you require.
• Requests can take up to 21 days to process.
• Bring a urine sample to the surgery before 11 AM.
• Complete a short symptom form to assist the nurse in assessing your condition.
• Antibiotics are not routinely prescribed and may require further testing before treatment.
