The family General Practitioner is the first port of call for the sick, but additionally people are now seeking help and advice for all sorts of family and social problems. This need is steadily increasing, the public are seeking to utilise services that they can trust and rely upon, should the need arise. e-Consult Update

Econsult Will Be Unavailable Over The Weekend


We will be switching eConsult off every weekend, while the surgery is closed and you will be unable to send one from 6.30pm on Friday until 8am Monday morning. This is to ensure that our patients seek more appropriate healthcare whilst we are closed.

You can use the eConsult facility to look up general medical advice. You can also get advice from the NHS website or the NHS App. Your local Pharmacist can help you with self care.

If your medical problem cannot wait until the surgery is open please contact the NHS 111 helpline by dialing 111 from any phone, or by visiting NHS 111 Online. NHS 111 online is also a good place to check your symptoms.

For anything life threatening call 999.

If you have received a letter from NHS England inviting you for your covid vaccine then please follow their guidance and they will offer you an appointment at a Mass Vaccination Centre.

The Yadava Practice fully support people with hearing problems, is disabled friendly and has access ramps into the Practice. If you need any assistance please don't hesitiate to contact us.

To book an appointment please check our Apointments page for full details. Please note there are no pre-bookable GP appointments available, you can either use our online appointment booking service or call on the day to arrange an appointment. Our booking lines open at 9am, please call any morning to be put on a telephone triage list in which the GP will return your call throughout the same morning. If the GP feels they need to see you they will offer you a Face to Face appointment.
Nurses are offering a mixture of telephone consultations and Face to Face appointments. These are Pre-bookable so you will just need to call the Surgery to book.
You can download a SSP form which you can give to your employer. This will cover you for the first 7 days of your absence. If you feel you need longer then you need to call before the 7 days is up to book a telephone triage with the GP who will be able to give you a MED3 which will entitle you to a longer period off work. This is a legal document and cannot be pre-dated or post-dated therefore it is the patient's responsibility to call within the correct timeframe.
Call the Surgery before 10am to request a Home Visit. Please remember home visits will only be authorised for those patients that are housebound and within the RM17 postal code area. We also have a Paramedic who works with the Surgery and it may be them that conducts the Visit.
Please send your request via email to prescriptions.f81211@nhs.net. Please allow 72 hours for this to be arranged and the prescription will be sent to your nominated pharmacy. Alternatively you can ask your nominated pharmacy to put the request in on your behalf. It is the patients responsibility to ensure the request is put in on time.
Please Download our Change of Address form and return it to reception. Alternatively you can pick up a copy of this at reception. Please note our catchment area is RM17, if you move out of area you will be required to register with another GP.
You can submit an e-consult which is available 24/7 - Access e-Consult now.
NHS 111 will be able to take your call after 6.30pm and at weekends.
Please call 999 in the case of an emergency or attend your local A&E
Please come along to the surgery where the reception team will give you a registration pack. Once this has been completed we will book you a new patient medical with the Nurse. Please bring two forms of ID along with you – Photo ID and Proof of Address, if you are registering a child then a copy of their birth certificate is required along with their vaccination record. PLEASE NOTE – WE ONLY REGISTER RM17
You would need to provide a copy of the proof of name change and we can update your record.
You would need to give consent for the practice to send SMS messages, this can be done by downloading the SMS consent form and either emailing it to the surgery or handing it to reception.

Please fill out a travel vaccination form, this will then be handed to the Nurse who will access your records and will then inform you if any and which vaccinations you would need. PLEASE NOTE THESE WILL NEED TO BE SUBMITTED 6 WEEKS PRIOR TO TRAVEL.FAILURE TO COMPLY WILL RESULT IN YOU HAVING TO ATTEND A TRAVEL CLINIC.
You would need to fill out a consent form and hand to reception. On this form you would need to detail exactly what information you need incl dates etc. This will then be handed to the Admin team and they will fulfil your request within 21 days.
NHS England require that the net earnings of Doctors engaged in the Practice is publicised, and the required disclosure is shown below. However it should be noted that the prescribed method for calculating earnings is potentially misleading because it takes no account of how much time Doctors spend working in the Practice, and should not be used to form any judgment about GP earnings, nor to make any comparison with any other Practice. All GP Practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each Practice. The average pay for GPs working in The Yadava Practice in the last financial year 2020/21 was £96,578 before tax and national insurance. This is for two full-time GPs who worked in the Practice for more than 6 months
Please bring a sample along to the Surgery before 11am. You will be asked to fill out a short form detailing your symptoms; this will then be passed to the Nurse to arrange the relevant treatment . PLEASE NOTE ANTIBIOTICS ARE NOT ROUTINELY PRESCRIBED FOR THIS IT MAY REQUIRE FURTHER ANALYSIS FROM THE LABORATORY.
